Health in Mind are delighted to launch our new online self referral function onto our website. Enabling you to access support directly without having to first have an appointment with your local GP or other health professional. You can still access via a health professional but we have launched this new facility so that you can act straight away and contact us to get the help you need without a moments delay. We will not pass any information onto your GP if preferred.
The self referral form has been designed to be easy to understand and gather all the information we need to arrange your first appointment. Once complete, your referral will be live on our clinical system and means we'll be able to provide you with a response and the help you need sooner.
We are a discreet service and you are able to self-refer through the link below without visiting your GP first. We will not pass any information onto your GP if preferred.
If you are a resident of East Sussex (Excluding Brighton and Hove*), are over 18 and you would like to refer yourself, you can use the link below.
This service is provided in partnership with our clinical information service provider (PCMIS based at York University).
